eForms

To reduce the number of paper notes submitted to school offices and to make it more convenient for our families, the district has created eForms that can be completed by parents/guardians online whether you are at home or at one of our schools. The forms are immediately received by our school offices. All schools offer attendance excuse, early dismissal, and tardy/late arrival forms. Additionally, the district has recently converted the request for an education trip form to an eForm. Links to the  middle school's eForms are below. Day-of Car Riders requests must be submitted by calling the middle school office.
